All international students with F-1 visa status are required to have injury and sickness insurance during their studies at the English Center. The English Center offers excellent insurance at a cost of US$22 per week. (See below for the additional cost to cover a spouse and/or children.) The insurance by United States Fire Insurance Company through Renaissance Insurance Agency, Inc.
The insurance may be used anywhere in the United States, but it provides the most coverage when the student uses one of its Preferred Providers (PPO). The Preferred providers in the Berkeley/Oakland area are listed below. The insurance covers up to $250,000 for each injury or sickness.
It covers
100% of most hospital charges at a PPO; 50% of customary charges at a non-PPO.
100% of most outpatient (office) charges at a PPO; 50% of customary charges at a non-PPO.
100% of customary charges for ambulance service to the nearest hospital.
100% of costs for pregnancy, including complications of pregnancy.

There is a $75 deductible. The student pays the first $75 of costs, and the insurance pays all other costs up to $250,000 per injury or sickness.
The following are not covered:
Normal health checkups
Dental treatment (except for injury)
Eye examinations (20% discount instead)

The cost of the insurance for a single person is $22/week. Additional cost for a spouse is $63/week; for one child $27/week; for two or more children $53/week.

If you prefer to use your own insurance coverage instead of the English Center's coverage: You must be able to certify to the English Center that your insurance covers at least US$175,000 per illness or injury. You will be required to provide a copy of this insurance policy to the English Center.
